Non-DOT Alcohol Test Forms are used to record results for any alcohol test that is not a Department of Transportation (DOT) regulated test. The non-DOT alcohol test form looks identical to the DOT alcohol test form, except all references to the U.S. Department of Transportation have been removed from the text. DOT regulations specifically state that non-DOT alcohol tests should not be recorded on DOT alcohol test forms. The non-DOT form should be used for breath or saliva alcohol tests that are not covered under the DOT testing program.
